If it was the USS Fitzgerald I’m remembering much of its bridge crew was made up of sailors from a different ship (a guided missile cruiser) which were serving on board while their own ship was undergoing repairs. And Cruisers and Destroyers not being the same ships and the replacement crew not having been trained on destroyers made a bunch of mistakes like accidentally transferring control to a different station without knowing it. In the end it the junior and senior officers will get punished even though it’s really the result of pushing heightened operating tempos for years while simultaneously not keeping up with maintenance or training in the military (especially within the Navy’s 7th fleet).
